I'm surprised I haven't heard/read more about this. A HUGE safety-related mistake, no doubt, and it wouldn't have taken much to turn from surprising to horrific.
Someone made a decision and someone else was nearly killed. Seems to me someone well up the decision making ladder ought to answer for it (no one in a lowly position has the power to decide to start the race) but I expect the marshall himself or someone near him will be blamed.

Some track changes have been made at Sepang
Turn 15 (last corner) is now off-camber and should allow for more overtaking. Some of the run off areas are now gravel.
