Sometimes it's the smallest things that do you in.....

 

A buddy of mine used to work in a press, and one of the guys there was stacking rolls of paper with a lift. The paper rolls were stored on the second and third floors of the building, and he was placing one high on the stack. He nudged the lift just a little too far, and the one ton roll of paper went out the window, fell two stories, and landed on the boss' car.

My friend said the guy got off the lift, looked at the wreckage, walked to the entrance, clocked out, and no one ever saw him again.
